CONSULTING COMPONENT – preparing the video

Questions to ask yourself after each consultation:

  1. Do I know significantly more about this person as a human being than before they came through the door?
  2. Was I curious?
  3. Did I listen?
  4. Did I explore their beliefs?
  5. Did I make an acceptable working diagnosis?
  6. Did I use their beliefs when I started explaining?
  7. Did I share options for investigations or treatment?
  8. Did I share in decision-making?
  9. Did I make some attempt to see that my patient understood? • Did I develop the relationship?

PERFORMANCE CRITERAI FOR THE MRCGP VIDEO: 

  1. the doctor is seen to encourage the patient’s contribution at appropriate points 
  2. the doctor is seen to respond to cues
  3. the doctor elicits sufficient details to place the complaints in a social and psychological context 
  4. the doctor takes the patient’s health understanding into account * 
  5. the doctor obtains sufficient information for no serious condition to be missed 
  6. the physical examination chosen is likely to confirm or disprove the doctor’s hypothesis, or is designed to address the patient’s concern 
  7. the doctor appears to make a clinically appropriate working diagnosis 
  8. the doctor explains the diagnosis, management and effects of treatment 
  9. the content of what the doctor says and the language used is appropriate to the patient’s needs 
  10. the doctor explains using some, or all of the patient’s beliefs * 
  11. the doctor is seen to make an effort to confirm the patient’s understanding * 
  12. the management plan is appropriate for the working diagnosis, reflecting a good understanding of modern, accepted medical practice 
  13. the doctor share management options with the patient
  14. the doctor prescribes appropriately
  15. the doctor establishes an effective rapport with the patient

CRITERIA

 

 

 

ERROR/S

 

·     The presence of a single major error on the consultation or of a number of minor errors should lead to consideration of referral.

·     serious error = causes actual/potential harm

·     minor error    = inconvenience only

 

 

LISTENING

·     Identify and elucidate reasons for attendance.

·     A credible/ acceptable plan should be negotiated.

 

 

ACTION

·     Appropriate action to identify patient’s problems. 

·     Reasonable investigations/ referrals. 

·     Help sought when necessary. 

·     Patient’s problem should be managed appropriately.

 

 

UNDERSTAND-ING

·     GP Registrar understands process/ outcome of consultation.

·     Actions explained. 

·     Obvious shortcomings identified and relevant background mentioned.
